New Families Start Here
New Families - Looking to register at your home boundary school? Start with #1 below. Interested in a school that is not your home boundary school? Start with #2 below.
* Note: Open enrollment applications must be approved prior to registering for a school that is not your home school. If you are regsitering for your home boundary school, you DO NOT need to fill out an open enrollment application.

- New Parents: For parents new to CUSD (grades Preschool-12), click here to begin your child's registration process for your attendance boundary school. Registrations are being accepted for the current school year, 2024-25 and next school year, 2025-26.
- Open Enrollment: Families interested in their child attending a school other than their attendance boundary school will need to apply for open enrollment. Open enrollments are being accepted for the current school year, 2024-25 school year and next school year, 2025-26 school year.
If applying for an open enrollment school, please do not complete your child's registration for the Open Enrollment school UNTIL you have received your approval email or letter.

Required Information for Online Student Registration

You will need the following information to enter and upload into our secure Online Registration Portal:
- Household information – address and phone numbers
- Parent/Legal Guardian information – work and cell phone numbers, e-mail addresses
- Student information – demographic and health information
- Emergency contact – names and phone numbers
- *Proof of Residency – current utility bill, lease agreement, etc. (see Acceptable Proof of Residency under Additional Student Registration Information on the right)
- State certified birth certificate
- Legal guardian or legal custody papers (if the parent/legal guardian is not on the birth certificate)
Additional Student Registration Information

*Acceptable Proof of Residency
The document must bear the parent or legal guardian’s full name and residential address or physical description of the property where the parent or legal guardian resides with the student must be submitted online (no P.O. Boxes):
- Valid Arizona driver’s license, Arizona Identification Card or Arizona Motor Vehicle Registration
- Valid Arizona Address Confidentiality Program Authorization Card
- Real estate deed or mortgage documents
- Property tax bill
- Residential lease or rental agreement, including Section 8 agreement or off-base military housing (must be on letterhead of rental company)
- Current Utility Bill (Gas, Electric, Water, Cable, Phone) with name and address. Disconnect notices will not be accepted.
- Bank or Credit Card Statement
- W-2 Wage Statement
- Payroll Stub
- Certificate of tribal enrollment (506 Form) or other identification issued by a recognized Indian tribe in Arizona
- Documentation from a state, tribal, or federal government agency (Social Security Administration, Veterans' Administration, Arizona Department of Economic Security)
- Temporary on-base billeting facility (for military families)
- Residency Affidavit (see Affidavit of Shared Residence below)

When the school offices reopen, you will be required to show an updated Proof of Residency document. A second Proof of Residency may be requested at any time.

Birth Certificate
When the school offices reopen, you will be required to show your child’s original Birth Certificate. If the original Birth Certificate is not submitted upon the day of registration, the original must be submitted within 30 days.

Immunization Records
When the school offices reopen, you will be required to bring in your child’s COMPLETE IMMUNIZATION RECORDS WITH DATES to comply with Arizona State Law. Please visit the Health Services Department for more information.

Academic Record
The school office may contact you for one of the following:
- Withdraw form
- Report Card from prior school
- State test scores - High School ONLY
- Transcripts from prior school - High School ONLY

Legal Guardianship or Custody Papers
If applicable, when the school offices reopen, one of the following must be submitted:
- Current, Valid Court Order
- Arizona Court Appointed Guardianship Papers
- Documentation from Superior Court of Arizona showing the pending court date for Guardianship hearing. Final papers must be provided within a week of the hearing date.

Special Education
Contact the school if you have a current copy of the IEP and current psychological report for your child.
