    		How to Apply:
    • Complete the Elementary Open Enrollment Application.
    • Complete the Elite Performance Academy Supplement Form.
    • Attach student documents outlined in application (grade report, discipline and attendance records).
    • Obtain Practice Facility Owner/Coach signature on the Supplement Form for verification of practice days and times.
    • Submit Supplement Form with all documentation to:

    Registration Registration
    • Elite Performance Academy administration receives, reviews, and approves applications throughout the school year.
    • Welcome Letter sent to approved families with orientation and tour dates.
